We are open to serve you, answer questions or schedule an appointment during the following hours:● Monday, Wednesday, Friday: 9 a.m. – 5 p.m. Closed for Lunch from Noon to 1:30 p.m.

Office Information

Return visits are generally 30-60 minutes in length. The fee for a straightforward return visit of 30 minutes will be approximately $167 plus tax. Payment is due at the time of service. We accept payment by check, cash, VISA, AMEX or MasterCard.

We are a participating provider for HMAA. Patients with some HMSA plans, and with UHA, pay at the time of service and are provided a receipt and insurance claim form (Super Bill) to submit for reimbursement by their plan.

Dr. Traub will provide most, if not all, of the products/medicines that he recommends for you. If you wish to request refills, please contact the office by email, mail, or fax the office at least one day prior to when you wish to pick up or have the package mailed to you. NOTE: We do not accept refill requests by phone or by voicemail unless you do not have the ability to email or fax your request.

Email is preferred for general questions and updates.

Phone calls in excess of five minutes may be billed as phone consultations.

Email between you and me is intended to make communication easier for both of us and might make your experience as my patient more pleasurable, informative and efficient. Keep in mind that email never replaces face-to-face communication.

This email address is utilized only for patient care. This is not a personal email account. No-one else besides me has access to this email account. Without your consent, your email address will never be provided to a third party not affiliated with our private practice.

Correspondence should be brief and concise, and completely related to your medical care. I will not be able to respond to complicated questions that require a lengthy response. If you ask such a question, I will reply that we need to discuss such a question or issue in person at your next appointment. Furthermore, non-medical questions or issues such as billing questions may be forwarded to members of my office staff that are better suited to answer them. Please include your full name and date of birth in every correspondence so that I may verify your identity.
Email fee policies:

Email correspondence between health care providers and patients is now a billable service that may be covered or reimbursed by health insurance plans. The criteria for billing is as follows:

Email may be billed if the time spent by me is more than 5 minutes over a 7 day period. For example, if you send several messages during a 7 day period that takes me more than a total of 5 minutes, you may be billed.

For email messages between 5-10 minutes, the fee is $15.

For 11-20 minutes, the fee is $30.

For more than 20 minutes, the fee is $50.

Examples of messages that will be billed include those that require medical decision-making on my part (e.g. diagnosing/treating a urinary tract infection), questions about a photo you send of an injury or a skin condition (e.g. a rash), questions about medications and supplements, and assessing your response to medications and lifestyle changes.

Email is inherently an insecure manner of communication. Email messages typically travel though many other computers and servers before arriving at their destination. At any point along this pathway, these messages can be intercepted and read by a third party. Additionally, if you share your computer with members of your family or friends, or you use a professional email account that could be accessed by your employer or colleagues, then transmitted information might not be confidential. Therefore I can not guarantee the confidentiality of the messages you send me, or my messages sent to you.
I am not responsible for any email or information lost due to technical failure.
You should not utilize email for urgent messages or any matter that requires immediate attention. Any urgent issues should be conveyed by calling the practice directly at 808-329-2114.
All email correspondence will be incorporated into your permanent medical record. After this has been done, I will delete all correspondence from my computer and from the server I use.
Unless otherwise noted in an auto-reply message to you, I will reply to your email within 72 hours. If you do not receive a reply within this timeframe, please call the office at 808-329-2114
If you are not satisfied with my reply, please make an appointment in my office so we can more thoroughly answer your questions and address your concerns.
I will not respond to questions from a person whom is not an established patient of mine. Please do not provide this email to such a person.
Failure to comply with any of the above policies may result in a termination of your email privileges.

Fees

Costs for initial visits generally range from $277-$369 plus taxes.
The fee for a 30 minute visit is approximately $167-$231 plus taxes.
Phone call and telehealth appointments are available and fees are the same as in-person office visits.
Fees are adjusted annually.

Good Faith Estimate Disclosure: you have the right to receive a “good faith estimate” explaining how much your medical care will cost. Under the law, healthcare providers need to give patients who do not have insurance or who are not using insurance an estimate of their bill for medical items and services. Fees are discussed at the time that an initial appointment is scheduled, and an estimated cost is provided.
